Reports to: Clinical Manager Essential Job Functions and Responsibilities: Provides physical therapy services to patients according to a written physician’s plan of care period Follow conditions of participation Work within the scope of Physical Therapy Coordination of patient care Performs other duties as assigned Position Qualifications: Licensed to practice as a physical therapist with through the state Preferred 2 years of experience as a Physical Therapist No Home Health experience is required Possesses and maintains current CPR Certification Driver License and Auto Insurance is required
